The comprehensive assessment guides the resident and clinicians in developing an effective person-centered treatment plan. The treatment plan addresses medical/biological needs, mental health, nutritional guidance, spirituality, emotional health, physical exercise, life skills, and ongoing support.
Recovery Continues After Residential Treatment
An important component of any successful recovery plan is a transitional/aftercare plan that includes support systems, plans for continued recovery, and relapse prevention. The plan lists specific steps to continued recovery as well as short and long-term goals.
Safe Harbor Recovery Center uses Trac9 relapse prevention assessments to manage and plot individual recovery progress. Clients who use Trac9 benefit from a state-of-the-art program that helps them maintain sobriety like no other program can. This benefit lasts well after the residential program is completed, as anonymous post-treatment surveys are sent each month for 12 months after discharge.
During your time at Safe Harbor, you will complete Trac9 assessments each week to allow the treatment team to monitor your improvement. Your responses will be used to generate topics for therapy sessions, allowing your counselor to target your specific needs and thus proactively reduce relapse risk.
Treating Co-Occurring Mental Health Disorders
